Please notice that the information forms below. Click on the form that corresponses with you service to open the file. You have the option of printing these forms out prior to your appointment or you may arrive 20 minutes early to fill them out at the SPA.

1. The first Client Intake Form is a MUST, that is to be filled out by everyone that has services rendered in our SPA.
2. The second form is a consent that the STATE REQUIRES us to have. It is only for massages.
3. The third form is for anyone that is receiving a Skin Care service. If you are receiving Clinical Facials you will have additional forms to fill out at the SPA.
